TOURNAMENT APPLICATION FEE

The following items are required at Check-In:

1. Official Roster - 1 Copy (downloaded from the FYSA website, or an FYSA signed by and ADC/DC)
2. Tournament Roster - 3 Copies (please complete, including regular players AND guest players)    NOTE: Not required to be signed by Registrar, DC/ADC, or Club representative.
3. Medical Releases - (provided below if needed) Only required at check in
4. Guest Player Forms - If necessary (6 guest players maximum allowed)
5. Player Passes
